
341 Meeting of Creditors
After you file your Chapter 13 Bankruptcy petition and plan, you will be required to attend a Section 341 meeting, which is an examination under oath conducted by the Chapter 13 Trustee. The U.S. Bankruptcy Court will set the date for this mandatory appearance.
In accordance with U.S. Trustee (UST) policies, the Trustee is conducting all 341 Meeting of Creditors using the Zoom Video conference program.
Parties should navigate to https://www.zoom.us/join, click “JOIN”. Enter Meeting ID: 6404983574, and Passcode: 5580468124
Debtors and their Attorney must appear by video. You should be in a quiet place that is free from distraction. Parties may not record audio or video or take photographs of the 341 meeting. The Trustee will not conduct your meeting of creditors if you are driving a motor vehicle. It is the Trustee’s intent to allow all parties to be present in the meeting room. As a result, is it important that parties mute their microphones until you are asked to respond, or your case is called. The Trustee also recommends that parties leave their video off until their case is called or until the Trustee or Staff Attorney for the Trustee, asks that the video be turned on. If a party needs to get our attention, turn on your video. Then you can un-mute your audio and speak when called on. If parties are disruptive during the meeting, they may be moved to the waiting room or removed entirely.
If a party loses the Zoom connection, reconnect in the same way you logged in initially. If you are unable to reconnect, please contact your attorney immediately and let them know what happened.

To conduct each meeting, the Trustee must be able to identify the debtor through authentication of a government issued picture identification and verification of the debtor’s Social Security Number. To do so, prior to the meeting (at least 14 days) all debtors must submit to the trustee in the secure manner described below: (1) a clear copy (in color) of their original government-issued photo identification and (2) confirmation of their Social Security Number. Acceptable forms of photo ID and proof of SSN are listed below. Do NOT display ID documents at the meeting.
Debtors represented by counsel must provide proof of your identification and Social Security number. Your attorney will need to provide to the Trustee high-quality copies of these documents at least 14 days prior to your meeting of creditors. Counsel for the debtor MUST submit this information through the secure Document Delivery Portal. If your attorney did not obtain these copies from you when you filed your case, contact your attorney immediately. DO NOT EMAIL OR TEXT THESE DOCUMENTS TO THETRUSTEE OR HER STAFF.
Acceptable forms of identification include: driver’s license, U.S. government ID, State ID, student ID, passport (or current visa, if not a U.S. citizen), military ID, resident alien card, or identity card issued by a national government authority. Acceptable proof of your Social Security number includes: Social Security card, medical insurance card, pay stub, W-2 form, IRS Form 1099, or Social Security Administration statement.
Additionally, at least seven (7) days before the meeting, we must have received (via upload through EPIQ’s Document Delivery portal) the following documents: (1) copy of your tax return for the most recent tax year ending immediately begore the commencement of the case and for which a return was filed, including any attachments, or a transcript of the return; (2) or for self-employed debtor(s) a completed Business Questionnaire and copies of the tax returns for the past 2 years. Additionally, pay advices or an affidavit of no pay advices must be filed on the docket.
Pro Se debtors (those not represented by an attorney) will be required to present valid identification to Trustee staff prior to their meetings of creditors. Pro Se debtors will also have to coordinate their means of participation for the meeting of creditors prior to the date of the meeting.
If the debtor fails to provide either an acceptable identification document or Social Security Number document to the Trustee prior to the 341 meeting, the Trustee will likely adjourn and continue the meeting of creditors or lodge an objection to confirmation.
Debtor(s) should not display any personal identification documentation to the camera or otherwise disclose any personally identifiable information like their social security number, phone number, or address. The debtor(s) should have copies of their bankruptcy statements, schedules and plan with them so the same can be referred to during the meeting.
At this meeting, the Trustee will place you under oath and ask you questions regarding the content of your proposed bankruptcy plan, your schedules, assets, debts, and other matters. After the Trustee is finished, your creditors will have an opportunity to ask you questions regarding the location and condition of your property, hazard insurance coverage, and other matters related to your financial affairs.

Creditors should raise their hand if they wish to ask questions. The Trustee will also ask if there are any creditors in the cases called. Please do not speak until you are called upon. Before asking your question, please state your name and who you represent. If able to, please rename yourself before the meeting on Zoom to identify who you represent.
